The presentation will address tools for optimizing electronic health record data to incorporate the standardized pain information model. Participants will learn about and participate in exercises to write a problem statement and smart goals, create a fishbone diagram addressing problems/ barriers, and break down change steps in a project plan to track progress.

Objectives:

• Identify processes and tools to optimize the implementation of a new pain assessment tool.
• Demonstrate tools used to prepare for successful implementation of a new pain assessment tool. 
• Understand the balance between standardization and customization as they pertain to patient-centered care, clinical practice, and documentation.
